How to remove yourself

    At usphonebook.com/removal (the US Phone Book Opt-Out Form), choose whether you are the subject of the request or an authorized agent.

    Enter your first name, last name (middle name is optional) and email address. Tick the authorization box, complete the reCAPTCHA and click "Begin Removal Process." USPhoneBook then sends a link to your email.

    Click the emailed link

    The email may take some time to arrive. If you wait more than 24 hours to click the link, you need to request a new one.

    Fill in the removal form

    Enter your information so that it matches the record you want opted out. Leaving fields out or entering inaccurate details only hinders the process. You then see a confirmation page and receive a confirmation email. USPhoneBook says to allow 3 days for the request to be fully processed.
